Hi. I see this word around the forums a lot, but I do not know what it is or what it means. Can someone enlighten me?

It is a form that you fill out with personal information such as address, #ss and so on. It also for tax purposes. You usually sign an Affy for wins over $500 so they can show that you agreed to pay the taxes on the prize. How this sounds right.

you of all people should know how google works, lol

its a signed statement of you verifying your identity to be able to collect a prize - they want to know its you who entered. and it usually needs to be notarized. and not always, but they're usually for the bigger wins because they want to get your social security number for tax purposes.
