I was just wondering what this means.......

There are a few Websites (popular cable channels) that I noticed this URL when I complete my entry... (I've used different computers)

Does anyone know what viral form means? :frown4: I'm thinking it's not good.

It means it's "hot" as in viewed a lot and very popular

I believe it's referring to viral marketing/advertising. After you submit your form, you might have gotten a page asking you for email addresses to forward the sweep to. That would be the viral form it's referring to. Viral marketing is like word-of-mouth, social advertising. The concept is old, but the term is more modern. The earliest "viral marketing success" I can recall was the Blair Witch movie. It got huge despite the lack of typical ads early on.
